Submitting a request
Complete the Credit Inquiry Request form (PDF*) including account numbers and customer authorization section. Please provide full account numbers with your request. Include your return fax number or email address on the form so we can return our response as quickly as possible. If a fax number or email address is not included on your request, our response will be mailed to the address if provided. For faster processing, please complete the form on your computer before printing.
