Self-Certification forms and instructions

This page contains required Self-Certification forms for Wells Fargo. These forms are used to collect tax residency information from customers as required by the new global tax initiatives, UK CDOT (United Kingdom Crown Dependencies and Offshore Territories) and CRS (Common Reporting Standard). CRS is a multi-year tax requirement that has been approved by over 90 different countries.

A Self-Certification form must be completed in order for Wells Fargo to comply with the tax reporting requirements of the local jurisdiction where accounts are held. In some cases, an additional form (Addendum) may be required. Instruction documents are provided for each form. The instructions will help determine if an Addendum is needed.
